Sprout can automatically generate or update a Transition Plan using the learner's notes, stories, certificates, and existing plan data.
Step 1: Open a new Transition Plan
You can open a blank Transition Plan in one of two ways:
Option 1: From the Users table
Go to the Champion Dashboard.
Locate the youth in the list.
In the Transition Plan column (far right), click Create Transition Plan.
Option 2: From the Plans page
Go to the Plans page. Click Create New
Select the appropriate Transition Plan template.
Select the Learner you are creating the plan for.
Step 2: Click Generate Plan or Update Plan in Sprout
On the Transition Plan page, click the Sprout icon. (lower-right corner of the screen)
In the Sprout panel, you will see one of the following buttons:
Generate Plan – If this is a brand new, blank plan.
Update Plan – If a plan already exists or is partially completed and needs improvement.
Click the button shown to begin.
Step 3: Sprout gathers context
Sprout will begin gathering relevant information, including:
The learner's profile
Notes
Stories
Existing plans
Group template guidance
Step 4: Provide additional context
Sprout will ask you to confirm or select which information you’d like included in the Transition Plan.
You will be prompted to:
Select stories to highlight in the plan.
Include or exclude certain notes.
Provide additional context in the message field
This allows you to control what is considered when generating the plan.
Click Submit to generate the content.
Step 5: Review and Publish the Plan
Review the generated content carefully.
Make any edits or adjustments if needed.
When you’re satisfied, click Submit to apply the changes to the Transition Plan.
Click Publish Plan to finalize and publish it.
Important
Sprout provides a draft based on available data and the group template. Always review the content for accuracy before submitting and publishing.
