Transition Plan (formerly called 'Portfolio') is used to showcase the member's achievements, completed certificates, and published stories. It effectively highlights their milestones and plans to move forward as part of the outtake process.
Here's how to build a Transition Plan:

Access the Champion Dashboard. Click on the Plans tab on the left navigation menu.

- On the Plans page, click Create New and choose Transition Plan from the available templates.
- Choose the member for whom you are creating the transition plan.
- Create a title for the Transition Plan and fill in the following details:
- Select Certificates: You may either select the same certs included in the growth plan or choose new certs from the dropdown. (To know more about assigning certificates, see article: How do I assign Certificates (certs) to Learners?)

Highlighted Stories: Choose the stories that you want to show on the document.
- After selecting the stories, AI Insights may be used to generate coaching feedback to fill in the boxes for Celebrating You and Moving Forward.

- Celebrating You: Share some details about the areas where the member has shown significant progress.
- Moving Forward: Provide some practical recommendations that will help them improve further.

Goals and Activities: Set additional tasks that will help them grow (optional).
- On the Signatures section, the names and signatures of the people who have been engaged in the member's growth plan may also be added.
- If you need to send a copy of the plan to someone and obtain their signatures, refer to this article: How do I share a Plan with a guardian.
- If unable to obtain signatures, you may affix your initials on behalf of that person by checking the box for "I have permission to sign for this recipient".
- Choose to either Save Draft or Publish the plan:
- Save Draft: The plan is just in draft and will not assign the journeys to the member yet. Each time you make changes to the plan, but not ready to finalize yet, make sure to click on Save Draft.
- Publish: Once the plan is final, click on Publish button. This makes the plan downloadable and visible on the member's profile. Only after publishing can you request signatures.
- Note: If you want the learner to see the plan, check the option "Allow member to view published plan" before clicking on Publish.
Convert Growth Plans to Transition Plans
An easier way to create a transition plan is to convert an existing growth plan.
When an existing growth plan is converted to a transition plan, any assigned journeys, additional activities, and names in the Growth Plan will be carried over to the Transition Plan.
To convert a growth plan to a transition plan:
- Go to the Plans tab in the Champion App and search for the member.
- See under the Template column if there is a growth plan that was previously published for the member. Click on the three dots and choose Convert to Transition Plan.
- At the top of the current list, a new line item with the title "Converted from [name] plan" will be generated. Click on the three dots beside it to edit.
- Proceed to edit the plan by following the Steps 4 - 6 under How to build a Transition Plan.
